LA CYGNE – A shift in health services is already in process at the community-owned medical clinic in La Cygne. Olathe Health Family Medicine ended its occupancy of the clinic on Saturday, Sept. 24, and the new health care provider is preparing to move in.
Community Health Center of Southeast Kansas (CHC/SEK) officially opens its newest medical clinic in La Cygne next Monday, Oct. 4, in the facility formerly operated by Olathe Health at 1017 E. Market St. The clinic will expand hours and services including Walk-In Care.
Patients wishing to establish care with CHC/SEK can begin the process now by completing a new patient packet available at the clinic, or by completing it online by clicking “La Cygne New Patient Forms” at https://chcsek.org/patient-forms/.
The completed forms may be returned to the clinic, mailed to CHC/SEK at P.O. Box 1832, Pittsburg KS 66762 or faxed to (620) 231-5062.
CHC/SEK expects to take possession of the building on Thursday, Sept. 30, and open the following Monday. Initially, the new CHC/SEK clinic hours of operation will be 8 a.m. to 5 p.m. Monday through Friday.
Karl Kroen, APRN-C, will continue to provide care in La Cygne location as part of CHC/SEK. The clinic will continue to provide laboratory and X-ray services.
The medical records of patients of Olathe Health Family Medicine in La Cygne will remain with the Olathe Health system. For patients wishing to transfer their records from Olathe Health to CHC/SEK, it is necessary to sign a transfer form, which is included in the new patient packet.
On Oct. 18, CHC/SEK will begin offering Walk-In Care (with no appointment necessary) as well as expanded clinic hours from 7:00 a.m. to 7:00 p.m. Monday through Saturday.
